Countryside Health Care Center has prided itself as a privately-owned skilled nursing and rehabilitation center that offers a wide range of care options for post-hospitalized patients. Some of our top service offerings include Reliant Rehab therapy, short-term and long-term skilled nursing, and respite care. Countryside Health Care Center is conveniently located near you and your family. We are just off of Highway 20 in the center of Sioux City. However, we are available to families located across the Siouxland areas.
